Your first visit to Helyx Health is a welcoming step where you’ll discuss your needs with our compassionate professionals who will help create a personalized plan for your well-being. For more information, contact us or request an appointment online. We provide both in-person and virtual appointments. We serve patients across the state of California including locations in San Francisco, Palo Alto, and Beverly Hills, CA.

What essential information should patients know before they arrive? (e.g., parking, office location details, policy)

• Palo Alto Office: Our office is located at 518 Hamilton Ave Palo Alto, CA 94301. Street parking is available.
• Los Gatos Office: Our office is located at 825 Pollard Rd, Ste 100 Los Gatos 95032. There is parking lot available at the building.
• San Francisco Office:  Our office is located at 57 Post St Suite 604 San Francisco, CA 94104. Please enter the building and take the elevator to the 6^th floor. Walk straight ahead, where you will see our office. You may come in and sit at waiting area, where your provider will meet you.
– Street parking is limited; nearby parking garages include: 173 Sutter St and 223 Sutter St.
– We are accessible by BART and Muni trains at the Montgomery St Station, as well as multiple bus stops nearby.

• Telehealth Appointments: Your provider will give you an online meeting link.

What documents or items do patients absolutely need to bring to their first visit? (e.g., ID, insurance card, referrals, medication list)

• Patients will fill out the Helyx Health Intake Packet before their first visit, which will request all required information.

What are the first steps a patient takes before the visit? (e.g., scheduling a consultation, filling out digital paperwork in advance, confirmation calls)

Who will patients meet and interact with during their first visit? (e.g., front desk staff, MA, physician)

• Patients will meet with their scheduled provider. At our San Francisco office, patients may be greeted by front desk staff.
